How do I access Trans Health Klinic?

You can ask your primary care provider for a referral to THK, or you can refer yourself.

To refer yourself to THK, fill out our intake form. Then, send it to our location at 167 Sherbrook St. You can do this in person or by mail.

Once we receive your referral, you will be added to the THK intake waitlist. This waitlist varies between 8-12 months.

You can also contact your primary healthcare provider, such as a nurse practitioner or doctor, to access THK. This ensures you get full support during and after your transition. If you do not have a primary care provider, please contact Family Doctor Finder:

The GDAAY program offers gender-affirming care for youth 16 years of age and younger.

Peer support

Through first-hand experience, our peers can offer resources and guidance for anyone exploring their gender identity and transition. The team also does outreach, community-building, and advocacy. Our goal is to make sure no one feels alone in their journey. Some of the programs you can access through our peers include:

  • Parent group: drop-in support group for parents of trans folks.
  • Binder exchange: we provide binders and take donations of binders in good condition.
  • ID support: assist with navigating legal name change and gender marker change.
